  • Blue (citrate tube)
  • Red
  • Yellow SST (serum separator tube)
  • Green (heparin)
  • Lavender (EDTA)
  • Grey (sodium fluoride)

COMMON PROBLEMS
Collection Errors:
  1. Tubes need to be collected in FULL
There are additives in many of the tubes and if they are not collected in full, it will not reach the correct/optimum concentration.
  2. Wrong Tube
  3. Blood was hemolyzed
Paperwork/ID Errors:
  • Patient has WRONG NAME/SEX/DOB etc. on lab sheet:
In the COMMENTS section. Leave a note correcting the name/sex/DOB/etc. and stating it was incorrect previously.
When the lab notices this error, their corrective action is to call the provider to confirm. Since most clinics are closed by the time the lab calls, they can't confirm the lab and its stuck in limbo. Your note in the comments section will serve as a confirmation of the correction any previous discrepancies
